Medical Release Forms
Annual Medical Release Forms must be filled out and notarized in order for your child to leave campus for any church event and or to participate in Choir Tour or Summer Quest.
Notaries are available at the Kid Quest desk on Sundays and Wednesdays. Please have the form completed before attending the first event.
L.I.T. Applications and Contracts
Must be completed and signed by both parent and child before participating in the program.

Choir Forms
CPF Forms (Child Protective First)
CPF Forms are required on any person working with children and students under the age of 18.
You can pick a copy up at the Kid Quest and/or the Front Office desk.
